Employer-sponsored grocery stipends are moving into the pharmacy benefit stack. True Rx Health Strategists, the Indianapolis-based pharmacist-led pharmacy benefit manager (PBM), announced Wednesday it has become the first PBM to integrate Instacart Health Fresh Funds into a clinical employer benefit program, giving plan sponsors a structured way to fund nutritious food purchases as part of weight management and diabetes care.

The partnership, set to launch in January 2027, will be available to True Rx employer clients whose members are enrolled in either the True Outcomes: Weight Management or True Outcomes: Diabetes Management programs. Employers select a monthly allotment of $50 or $100 per participating member; funds refresh each month for up to six months, contingent on continued program enrollment.

The Clinical Framework

The food-as-medicine model underpinning the arrangement ties grocery access directly to dietitian oversight. After completing a qualifying session with a True Rx registered dietitian or clinician, members receive an Instacart Health Fresh Funds code redeemable across any grocer in Instacart's network of more than 2,200 retail banners representing nearly 100,000 stores. Approved purchases span fresh fruits and vegetables, meats, seafood, and dairy. Dietitians track what members buy and use purchase data to steer follow-up clinical conversations — an accountability loop the company says closes the gap between a prescribed meal plan and the actual grocery cart.

In a six-month pilot of True Outcomes: Weight Management among members using GLP-1 medications, participants achieved weight loss of up to 15.0%, and 90.0% completed monthly virtual visits. True Rx CEO Adrienne Williams LaBorwit cited those results as evidence that pairing clinical guidance with concrete purchasing support drives measurable outcomes. "We can give a member excellent dietary guidance, but that guidance is more valuable when we also help make it actionable," she said.

Why Grocery Tech Meets the PBM

The timing reflects mounting pressure on self-funded employers from surging GLP-1 prescription costs. Semaglutide and tirzepatide have reshaped employer pharmacy spend, and many plan sponsors are weighing whether to cover weight-loss drugs at all. True Rx has responded by opening its True Outcomes: Weight Management program to any employee seeking to lose weight, regardless of whether their plan covers GLP-1 therapy — acknowledging that FDA labeling for those medications already calls for concurrent dietary modification and increased physical activity.

For Instacart, the deal extends the reach of its health-oriented Fresh Funds product into the employer benefits channel, a distribution pathway distinct from its existing partnerships with Medicaid managed-care plans and health systems. Sarah Mastrorocco, vice president and general manager of Health at Instacart, framed the arrangement as a scalable, measurable mechanism for employers to engage workers' nutritional needs alongside conventional pharmacy benefits.

In keeping with True Rx's transparent pricing model, employers are billed only for funds members actually spend. Aggregate utilization and clinical outcomes are reported back to plan sponsors, giving finance and benefits teams data to evaluate return on investment — a feature increasingly demanded by self-insured employers scrutinizing population health spend. The structure also ensures employer dollars flow only to members actively engaged with their care team, limiting outlay to high-intent participants.

The food-as-medicine sector has drawn widening attention from grocery retailers and foodservice operators as chronic disease management migrates toward nutritional intervention. Integrating stipend-funded grocery delivery into clinical weight-loss programs represents one of the more operationally concrete deployments of that framework in the employer benefits market to date.
